HOW DO I....
Obtain A Repeat Prescription?
Repeat prescriptions are on your computerised records which allow you to have a recommended number of issues without being seen by the doctor. You will be asked to be reviewed at least once a year even if you are under the hospital.
When you are issued with your medication, you will receive a computerised slip which has a list of all your medication. When you require a further supply please tick the boxes against all items required. Either post this to us or hand it in at the surgery, allowing 48 hours for this to be processed. Telephone requests are accepted between 10.30am - 5.15pm at Marmaduke Health Centre and 10.00am - 5.00pm at Orchard Park Health Centre. We will post prescriptions for you if you leave us a supply of stamped addressed envelopes.
If you have made private arrangements with your chemist, please allow them time to collect the prescription from the surgery and deliver it to your home.
Emergency prescriptions can be requested from 9.00am and will be ready after 2.30pm subject to the doctor's approval of issue.
THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
